THE OFFICES WORKING FOR STUDENT AFFAIRS AND SERVICES

The University serves the students in accordance with its mandate through the various offices which objectives and scope of work are described below:

A. ADMISSION SERVICES

  1. The office of the Director for Admissions administers the entrance examination to student-applicants as one of requirements.
  2. The University admits students who are of good moral character, have the required academic credentials and have qualified for admission.
  3. Coordinates closely with the College Deans on admission matters.

B. STUDENT RECORDS SERVICES

  1. The office of the Registrar shall provide basic academic information regarding curricular program, general admission requirements and other academic information.
  2. Maintains the academic records of all students.
  3. Evaluates the records of candidates for graduation and for honors.
  4. Issues certification transfer credentials, grades, diploma, and transcript of records.
  5. Issues permits for removal, completion, validating examinations, and student clearance.
  6. Sends copy of students grades to parents whenever possible.
  7. Issues a written notice on incomplete grades and/or other deficiencies every semester.
  8. Issues ID number and identification card at the beginning of the school year and validates the same during the 2nd semester.
  9. Issues official list of enrollment.
  10. Accepts from female students who get married during a school term an affidavit of change of status and a copy of the marriage contract duly certified and authenticated by the Civil Registrar of the City/Municipality, and effects the same in all pertinent documents of said female students.

C. LIBRARY SERVICES

  1. The Library provides the material and services to meet the instructional research and extensions needs of the various constituents of the University.
    1. Library resources include general references, books, periodicals, vertical files, and other multimedia resources.
    2. Special services rendered by the library such as the facilities e.g., audio-visual tapes, CD’s and internet, shall be charged reasonably fees.
  2. Library privileges shall be extended to a student upon the presentation of identification card issued by the University and duly countersigned by the University librarian.
  3. Students who wish to take out books for home reading will be required to secure a barrower’s card at the general circulation desk.
  4. The observed library hours from Monday to Friday is from 7:00 a.m. to 8:00 p.m. On Saturday some libraries are open from 7:00 to12:00 noon and 1:00 to 5:00 p.m. Libraries are closed on Sundays and holidays.
  5. A student is allowed to check out three books at a time from the circulation section. The student may keep the books for use for not more than two days depending upon the availability of the books. A graduate student is allowed a maximum of five books.
  6. Reserved books assigned by professors and instructors for course reading are for room use only and are barrowed, one at a time for one hour. Overnight loans are checked out during the last hour before the library closes. They must be returned the following working day between 8:00-9:00 a.m.
  7. Students with overdue will be fined reasonably as approved by the University. Destroyed books will be charged reasonably.
  8. A library book that is lost or damaged shall be replaced with a copy that is acceptable to the librarian. If the book is out of print, the barrower will submit an equivalent book with the approval of the University librarian.
  9. Smoking, eating and littering are strictly prohibited inside the library.
  10. Textbooks whenever available maybe procured and/or rented out to students at reasonable rental fees. The procedure of renting shall be governed by the guidelines to be formulated by the Supply office in coordination with the Cashier’s Office.
  11. Other rules and regulations regarding the use of library facilities being imposed by each college must be adhered to by the students.

D. HEALTH SERVICES

The Health Services Unit (HSU) offers medical/dental Services to the students.

  1. The HSU promotes the general health of the students.
  2. The HSU offers medical/dental services to the students as follows:
    1. Consultation
    2. Physical/medical examinations
    3. Dental examinations, prophylaxis and extension whether possible initial medication.
    4. Initial medication
    5. Issuance of medical/dental certificate for various services.
    6. Ambulance services.
  3. All students are required to undergo physical and dental examination annually.
  4. Students found with any contagious diseases shall be properly evaluated, managed and advised. The University medical officer shall be make appropriate recommendations/referrals.
  5. The HSU monitors campus sanitation, precautions and control of communicable disease and recommend appropriate actions.
  6. The HSU maintains the health records of all students.
  7. The HSU conducts orientations.
  8. The Medical/Dental Clinic shall monitor campus sanitation, prevention and control of communicable diseases and health education and make proper recommendation thereto.
  9. In coordination with the other agencies, the medical/dental clinic shall render referral services to students immediately in need of serious medical attention.
  10. Issuance of health certifications for various purposes.

  11. Follow-up cases.

E. FOOD SERVICES

  1. Services can avail of snack and meals at reasonable prices at the University cafeteria, canteen and the like.
  2. Safe drinking facilities shall be made available in the campus in strategic places.
