Advisory to all Incoming College Freshmen

Please be reminded that the submission of the following documents is a requirement to be included in the official list of enrolled students for AY 2020-2021, to wit:

  1. Original Copy of Form 138 (Grade 12 Report card)
  2. Original Copy of Good Moral Certificate
  3. Photo Copy of Birth Certificate (PSA)
  4. One Recent Passport Size Picture

We have extended the submission period until October 5, 2020. You may submit the abovementioned requirements in the designated drop boxes at Gate 1 and Gate 2 of the BulSU Main Campus. Kindly make sure that they are properly labeled (see below sample), secured in a sealed envelope.
