Advisory to All BulSU Applicants

ALL QUALIFIED BulSU APPLICANTS ARE REQUIRED TO SUBMIT THE HARD COPY OF DOCUMENTS LISTED BELOW STARTING MAY 1, 2021 UNTIL JUNE 30, 2021:

  1. Certified true copy Form 138 (Photocopy) Grade 11 (1st and 2nd semester) Grade 12 (1st semester)
  2. Certified true copy of Proof of Residency (Barangay Certification)
  3. Certified true copy of Certification or ID of the following if applicable:
    1. Member of an Indigenous Cultural Community (ICC)/Indigenous People (IP). - (Membership Certification/ Barangay – issued Certificate)
    2. Member of Pantawid Pamilya Pilipino Program (4Ps). - (Membership Certification/ Barangay – issued Certificate)
    3. Student with Special Needs (SSN) and other Type of Disabilities - (Membership Certificate/ Identification Card (ID))
    4. Child of solo Parent - (Solo Parent ID)
    5. With Exemplary Artistic and Athletic Ability - (Certification from the Heads of the school)

Note: Make sure that all documents are clear and readable.

The documents shall be placed inside a long brown envelope with plastic and with proper label at the back with the following information.

Important Note:
Applicants may choose to leave their documents at the drop box located at the BulSU Main Campus gates 1 or 2.  Alternatively, they may send the documents via courier addressed to the Office of Admissions and Orientation Services, Bulacan State University, City of Malolos.

Meanwhile, applicants in External Campuses may choose to leave their documents at the drop box located at the BulSU External Campus Gate or send them via courier addressed to Bulacan State University (Bustos, Hagonoy, Sarmiento, or Meneses Campus), whichever is applicable.

Deadline of submission is on June 30, 2021.
